A psychedelic swirl of deep blue and teal fills the entire label background, with a small silhouetted boat and figure in the lower foreground facing an enormous wave whose crest forms a grotesque mouth or creature face in shades of navy and cyan. The beer name 'FOG MONSTER' appears in bold white sans-serif type across the upper-middle section, with 'HAZY IPA' in smaller type beneath it. The composition creates a nightmarish seascape where the boundary between wave and monster face dissolves into the undulating, hypnotic background.
